Grants Program Overview

During the initial phase of its grants program, Finding Answers awarded approximately $7 million to 28 health care organizations around the country to rigorously evaluate practical and replicable solutions designed to reduce racial and ethnic health care disparities for patients with diabetes, cardiovascular disease, and depression.  Awards ranged from $100,000 to $275,000 for projects lasting up to two years.

The second phase of the Finding Answers grants program focuses on identifying what types of interventions can be successfully implemented on a broader scale and evaluating those interventions across numerous practice sites and settings. By evaluating a similar intervention in multiple sites, we will learn what contextual factors affect program success and what type of quality improvement infrastructure is needed to facilitate the implementation of disparity interventions into everyday medical practice.

Current CFP

Our fourth Call for Proposals was announced on December 17, 2009. We are specifically looking for innovative interventions that are likely to be replicable and sustainable, and aim to disseminate promising practices to health care organizations nationwide with a focus on dissemination efforts within the 15 Robert Wood Johnson Foundation Aligning Forces for Quality communities.

Previous CFPs

Round 1 CFP
During our first Call for Proposals, a total of 178 applications were received.  In December 2006, 11 sites were selected to evaluate their proposed interventions. These proposals included interventions involving community health workers, electronic health records, health literacy screening and use of culturally tailored DVDs designed to empower patients to become more active in their care. 

Round 2 CFP
During our second Call for Proposals, a total of 134 project applications were received. In May 2008, ten sites were selected to evaluate their proposed interventions. These projects included telephone and internet technologies, nurses, community organizations, and provider incentive programs. 

Round 3 CFP
During our third Call for Proposals, a total of 111 applications were received.  In September 2009, seven sites were selected to evaluate their proposed interventions.  These projects included patient incentive programs, at home blood pressure monitoring, telephone-based counseling, and decision support for physicians.
