Call for Proposals

Currently, we do not have an active Call for Proposals.

Round 1 CFP

During our first Call for Proposals, a total of 178 applications were received.  In December 2006, 11 sites were selected to evaluate their proposed interventions. These proposals included interventions involving community health workers, electronic health records, health literacy screening and use of culturally tailored DVDs designed to empower patients to become more active in their care. 

Round 2 CFP
During our second Call for Proposals, a total of 134 project applications were received. In May 2008, ten sites were selected to evaluate their proposed interventions. These projects included telephone and internet technologies, nurses, community organizations, and provider incentive programs. 

Round 3 CFP
During our third Call for Proposals, a total of 111 applications were received.  In September 2009, seven sites were selected to evaluate their proposed interventions.  These projects included patient incentive programs, at home blood pressure monitoring, telephone-based counseling, and decision support for physicians.

Round 4 CFP
During our fourth Call for Proposals,a total of 60 applications were received. In October 2010, four sites were selected to evaluate their proposed interventions in a least three different sites. These projects included remote medical interpreting, internet based depression prevention and intervention for youth, patient financial incentives, and point-of-care automated medication delivery.
