Concurrent Peer Review Visits to Reduce Cardiovascular Risk Factors

This project aims to reduce racial and ethnic disparities in health care by improving cardiovascular and diabetes management among minority and poor patient populations. We propose to implement and evaluate an innovative, but practical intervention: concurrent peer review (CPR). CPR incorporates a visit from a doctor’s clinical peer during a patient visit to review the diagnosis and treatment plan to ensure quality health care is provided to the patient. These visits are designed to focus physician and patient attention on optimizing cardiovascular and/or diabetes management and disease control at the point care is received. The intervention minimizes competing clinical demands and clinical “inertia”. Patients will be randomly assigned one CPR visit with a physician other than their primary care physician. The primary study outcome will include control of hypertension, diabetes, and lipids. Secondary outcomes will include a set of clinical process measures that improve cardiovascular and diabetes care. The intervention builds on existing resources at the Westside Health Disparities Collaborative. A research team from the University of Rochester helped create the study design and will lead both the quantitative and qualitative evaluation of outcome and process measures. Project findings will be disseminated to peers within the health care stakeholder community for implementation as part of their quality improvement strategies.
