Project Title Real Time Tele-Monitoring of Glucose to Improve the Management of Type 2 Diabetes in Primary Care Clinics
Project Description Adequate glucose control increases survival and enhances quality of life for Type 2 diabetes mellitus patients (T2DM). Real-time glucose tele-monitoring (GTM) allows physicians to quickly adjust therapy at a distance limiting clinic visits, important among disadvantaged and rural groups for whom transportation to and cost of visits are barriers. This project will evaluate GTM enhancement of diabetes care in a minority and economically disadvantaged population in a randomized intervention study within the construct of the National Health Disparities Collaborative, a HRSA-sponsored program promoting quality of care measures in chronic disease management. Intervention participants will transmit glucose levels via telephone to a central server, which will alert the patient’s nurse if the level is out of physician-prescribed range. The project will test whether intervention participants will achieve better long-term glucose control (HbA1c status) compared to controls on a self monitoring regimen. It will also evaluate the cost and acceptance of GTM in a primary care setting.
