Who is the intervention targeting?
Minority patients with Type 2 diabetes
What intervention is being evaluated?
Intervention participants will transmit glucose levels via telephone to a central server, which will alert the patient’s nurse if the level is out of physician-prescribed range.

A Federally Qualified Health Center (FQHC) in Mobile, Alabama

Why might this approach work?
Real-time glucose tele-monitoring allows physicians to quickly adjust therapy from a distance which limits clinic visits (important to disadvantaged and rural groups for whom transportation costs are often a barrier).
How will this intervention be evaluated?
A randomized intervention study within the construct of the National Health Disparities Collaborative, a HRSA-sponsored program promoting quality of care measures in chronic disease management, is being conducted. The project will evaluate whether intervention participants will achieve better long-term glucose control (HbA1c status) compared to controls on a self monitoring regimen. It will also evaluate the cost and acceptance of this intervention in a primary care setting.
