Quality Field Notes: Reducing Disparities

Recently, the Robert Wood Johnson Foundation published a new entry in its Quality Field Notes series, Reducing Disparities to Improve the Quality of Care for Racial and Ethnic Minorities. Quality Field Notes are a collection of tools and resources that spotlight innovative approaches with the potential to transform health care and provide models for reform. This equity-focused package spotlighted the innovative work that Finding Answers has helped implement in several communities and clinics across the country.

Over the past three years, Finding Answers has been working closely with clinics and quality-improvement collaboratives from Aligning Forces for Quality. The shared goal of these partnerships was to identify disparities in health quality and outcomes, and to create programs that reduce and eliminate those inequalities. Five Things to Remember About Reducing Disparities
This infographic presents five important things that health care organizations should remember when planning efforts to reduce and eliminate disparities. These five points draw upon lessons learned from eight years of research from Finding Answers, including 33 evaluations of innovative interventions, 12 systematic reviews of disparities intervention literature and six projects to reduce disparities in Aligning Forces for Quality communities.

Issue Brief: Reducing Disparities in the Quality of Care for Racial and Ethnic Minorities
Through the Equity Improvement Initiative, Finding Answers, the Center for Healthcare Strategies, and six clinics in Aligning Forces for Quality communities partnered todevelop patient-centered disparities-reduction. With technical assistance from Finding Answers, clinics were able to design and implement demonstration projects that incorporated patient perspectives from the very beginning. Case Study: Reducing Disparities In-depth coaching and technical assistance from Finding Answers helped staff at Crossroad Health Center in Cincinnati, Ohio design a intervention to improve blood sugar control among Spanish-speaking patients. As a result of the intervention, quality of care improved for patients, and patients’ blood sugar control was improved. In Detroit, the Greater Detroit Area Health Council decided to adapt one of the 33 grantee interventions funded by Finding Answers: an innovative peer storytelling intervention. That intervention, a peer-to-peer video storytelling intervention, was piloted first at Cooper Green Mercy Hospital in Birmingham, Alabama, where it resulted in decreased blood pressure among patients who took part in the intervention.
