Supporting Equity in the Medical Home (Webinars)


In February 2014, Finding Answers teamed up with the Centers for Medicare and Medicaid Services and the TMF Health Quality Institute to provide a series of four webinars on reducing healthcare disparities for practices participating in the Comprehensive Primary Care Initiative (CPCI). The webinars offer practical tips and examples to promote equity and reduce disparities for practices designing and implementing shared decision-making, patient experience, and care management programs.

Webinar 1: Introduction

Our first webinar of the series provides an introduction to the significance of health equity, encouraging organizations to address the specific needs of minority groups as part of their quality improvement work. Dr. Ian Bennett from the University of Pennsylvania and Kathleen Hall from Spectrum Health Services in Philadelphia offer insight into their work tailoring health services to diverse populations.

 

Webinar 2: Shared Decision-Making

Our second webinar discusses the benefits of engaging minority patients in shared decision-making (SDM) using decision aids and open-ended, patient-directed, culturally competent communication. We emphasize the importance of tailoring SDM programs and recruitment to specifically anticipate minority populations’ unique needs. Dr. Elbert Huang and Dr. Monica Peek share stories from their work at the University of Chicago.

 

Webinar 3: Patient Experience

Our third webinar offers guidance on how to identify disparities in the patient experiences of racial/ethnic minority groups using stratified data and patient input. We discuss the value of informing patients about the changes made in response to their input using inclusive and culturally-appropriate communication materials. Brenda Battle from the University of Chicago discusses her work incorporating equity into improving patient experience.

 

Webinar 4: Care Management

Our fourth and final webinar of the series provides tips on how to design a care management program that incorporates equity. We elaborate on what care managers can do to understand patients’ cultural norms and preferences, to appropriately advocate for their patients, and to guide them through the care system. We also describe a few important traits and skills needed to provide effective and equitable care management. Pam Gentry, Cindy Rose, and Dr. Hayden Bosworth from Duke University Medical Center share stories and recommendations from their work in the field.
