    Roadmap to Reduce Disparities

    This structured but flexible process for building a culture of equity and implementing equity interventions is a great resource for both clinic staff wanting to reduce disparities and those who teach others. By following the Roadmap, healthcare professionals are guided through a standardized process to address equity, even though the causes of disparities may vary across regions or patient populations.

    This comprehensive guide lays a foundation for TA providers to teach equity & the Roadmap in an effective and persuasive manner. The guide includes PowerPoint presentations, facilitator notes and Roadmap Exercises for each step, which will help individuals brainstorm and translate principles into action for their organization.

    This section offers lessons learned, best practices and examples of what other organizations have done to reduce disparities in health care. Resources include:

    Reducing Health and Health Care Disparities: Implementation and Best Practices for Health Care Organizations (EQuII Brief)

    This strategy overview shares the lessons learned of nine health care organizations that implemented the Roadmap to address disparities in their patient populations. It includes concrete recommendations that other health care organizations can use to improve the quality of their health care delivery systems and implement interventions to address inequities in patient care and outcomes.

    This table is a high-level, visual overview of the best practices that organizations have used to make equity a priority.

     

     

     

    *Technical Assistance (TA) providers will also find it helpful to be familiar with the resources in the “Implement Change” section, since those resources are geared toward their clients (people actually implementing equity activities).
