Real-World Examples
Finding Answers funded the evaluation of 33 interventions around the U.S. that aimed to reduce racial and ethnic health care disparities in the areas of cardiovascular disease, diabetes, and depression. We evaluated a variety of strategies in different health care settings. As explored in Roadmap Step 4, Designing the Activity, these detailed descriptions of real-life interventions can be a powerful tool for organizations designing their own disparities-focused quality improvement project. Our intervention sites were chosen during four Calls for Proposals. Grantee interventions are also available as a list organized by round of funding and geographic location.
